This is a self-serve credit card payment gateway for our patients.  If you need assistance with your credit card payment or you prefer e-transfer instead, two request forms are found below.
IMPORTANT RECEIPT INFORMATION - Your ONLY opportunity to have a receipt is to put your EMAIL ADDRESS in the Email box found in the billing ADDRESS INFORMATION at the checkout before you pay.  Please remember to do it if you want a receipt.
